The Process...

Thanks for your interest in Glacier Nurse Direct! Please follow these steps to make an appointment:

  1. Complete Intake Form: Below you will find intake forms for both IV therapy & our concierge nursing services. All new IV therapy clients must complete the intake form on our website & review the IV Education link located on our Services page under IV Hydration Therapy.  All other potential clients please use the concierge intake form to connect with us or call/email if your not sure.  Choose the appropriate form, complete, & submit.

  2. Review: Our nurses will review your submitted form & call you back to confirm it was received and ask/answer any necessary follow up questions to proceed with making an appointment. 

  3. Book Appointment: Once all necessary information is received from you, an RN will call to book your appointment during the initial phone call regardless of the type of service you are needing.  At that time, we also require payment authorization from you to hold your appointment.  We will meet your wherever you request as long as it is in a reasonable location & within a 70 mile radius of Kalispell. Clients outside of this travel area will only be accepted on a case-by-case basis  & a travel fee will be charged for any services provided. At your first visit, you will need to sign the required forms: 1) Treatment Consent 2) Billing Consent 3) HIPAA Consent

  4. Payment:  We will gladly accept all major credit/debit cards & cash at time of visit. We also accept FSA/HSA for IV Therapy!

  •  For IV therapy clientsPayment is due in full at or before time of service by paying online. If you fail to make your scheduled appointment with us as booked you will be charged a non-refundable fee of $50.  It is recommended you call at least 24 hours in advance if you need to reschedule.

  • For concierge clients:  Payment is due no later than by date of your appointment unless discussed previously with the nurse at GND. Due to the fact that we are a  'concierge' nursing service and care is based on individual needs, charges will be determined on a case-by-case basis. A $150 charge will be applied if intake requires an RN consultation visit for assessment & care planning purposes & will be required for potential clients seeking longer term care management.  Payment options will be discussed during initial phone call with RN. You will then have the opportunity to pay your invoice via email or in person.

*If you are an IV therapy client who wants to pay early after booking your appointment OR  purchasing a IV Package or gift certificate please proceed to the IV Therapy Payment section on this page and click on link to complete your purchase. We accept Paypal or debit/credit card. A nurse will contact you to follow up as needed depending on your purchase. Online checkout only for IV Therapy Clients.
